ESAIC Exchange – Cologne Experience
Participating in the ESAIC Exchange Programme at the University Hospital of Cologne has been one of the most rewarding professional experiences of my career. As an anesthesiologist with a strong academic and clinical background, I applied to this programme with the aim of observing advanced practices in airway management and regional anaesthesia within a high-volume, well-organised European hospital. I am pleased to say that my expectations were not only met, but significantly exceeded.
From the moment I arrived at the University Hospital of Cologne, I was welcomed warmly by the team. The entire staff was exceptionally friendly and helpful, and I felt fully supported throughout the programme. My mentor, Dr Nicolas Leister, played a key role in this. He was incredibly approachable and attentive, always taking time to explain the procedures in detail and answering my questions thoroughly. Thanks to his support and guidance, I was able to integrate quickly into the department and benefit from every opportunity offered.
One of the highlights of my experience was the chance to observe and, when appropriate, participate in the management of both anticipated and unanticipated difficult airways. The hospital has a robust and clearly defined system for airway assessment and planning, which makes it easier to understand decision-making processes and clinical reasoning. The structured approach to airway management was impressive, combining safety, preparation, and adaptability in a very effective way. These observations provided me with valuable insights that I hope to implement in my own practice.
In addition to airway management, I had the opportunity to observe a wide variety of peripheral nerve blocks performed under ultrasound guidance. The team’s skill in regional anaesthesia was remarkable, and I gained a deeper understanding of both the technical aspects and patient-centred decision-making involved in choosing the appropriate block for each clinical situation. These experiences not only enhanced my knowledge but also inspired me to further pursue education and practice in this field.
Another aspect of this programme that I truly appreciated was the structured, yet flexible, nature of the daily clinical schedule. I was able to move between different operating rooms and observe a diverse range of surgical specialities. This helped me understand how anaesthesiology techniques are adapted to different procedures and patient profiles, which is invaluable for broadening one’s clinical perspective.
Beyond the clinical setting, my time in Cologne during May and June was equally enriching on a personal level. The city is vibrant, green, and full of history. I had the chance to explore various parts of Cologne and engage with local culture. With a basic knowledge of German beforehand, I was also able to improve my language skills during my stay, which added an extra layer of immersion to the whole experience.
One of the key strengths of the ESAIC Exchange Programme is its emphasis on both clinical observation and cultural exchange. It creates an environment where visiting professionals are not just passive observers but are actively integrated into the daily routine of the host centre. I believe this is what makes the programme truly transformative.
Reflecting on the experience, I can confidently say that it has positively influenced both my clinical vision and personal growth. The system I observed in Cologne — from patient arrival in the preoperative area to transfer to the postoperative recovery room — was efficient, patient-centred, and well-coordinated. If I had the authority and resources, I would certainly like to implement a similar system in my own hospital.
To anyone considering applying to the University Hospital of Cologne as part of the ESAIC Exchange Programme, I can wholeheartedly recommend it — particularly for those interested in pediatric anaesthesia, robotic surgeries, and ultrasound-guided regional anaesthesia. The centre offers a rich variety of cases and a warm, collaborative environment that makes learning both meaningful and enjoyable.
I am sincerely grateful to the ESAIC for offering such a well-structured and inspiring programme, and to the team at University Hospital of Cologne for their hospitality, professionalism, and openness. I return home with new knowledge, fresh perspectives, and a renewed passion for my work — and I encourage others to take advantage of this unique opportunity.
